Families may apply at any time during the school year for the free and reduced lunch program. Forms should be returned to your child's school office or by mail to the Board Office address at the top of the explanation letter.

 

IMPORTANT: This file includes an explanation letter, meal application form, and a form to disallow sharing information with Medicaid or FamilyCare.  Please be sure to look at/print at all three documents.

This form allows the district to obtain Medicaid reimbursement for certain services provided to special services students.

This form must be completed by the parent/guardian and physician to allow our school nurses to administer medication to students during the school day.

This form gives the district permission to use your child's photo on our website, in district publictions, and in other local media. 

This form must be submitted for any use of school facilities, including auditoriums, gyms, fields, cafeterias, and other other rooms or areas.  Please submit the completed form to the office of the school you are requesting permission to use.

Use this form to request a copy of your MTHS transcript.

This page must be printed, signed in each space indicated and returned to your child's coach each season they participate in athletics.  A copy can be obtained in the main office, if need be.

 

This form is state mandated and must be signed by participant and parent for each season of activity. 

All parents and participants are required to review this document.  After doing so, please sign the Signature Page (a seperate form) indicating that you understand the information and agree to abide by the policies within.  Failure to do so will render your child ineligible for the season.
