This form is required for all athletes.  Parent/Guardian must complete the medical history questionaire.  The school physician will complete the medical exam portion at free school physicals, or you may have your own physician complete the form and submit it for review by the school physician.

This form is required to be signed by the parent/guardian for all athletes.
