What do I do if I want to play a sport?
Find out who is coaching your sport to let them know you are interested and ask what you need to do. Listen for date of team meeting.
I missed my team meeting, what should I do?
Find the coach and see what information packets were given and other pertinent information discussed at meeting.
I need a pre-participation physical (PPE), what are my options?
1. To have continuity of health care, you should schedule an appointment with your family physician to do the PPE well in advance of the start of your sports season. Remember to bring the school’s PPE forms to the doctor’s office. Please note that forms have been updated as of Jan. 2008 - do not use older forms.
2. If you don’t have a family physician or would like the school’s physician to provide this service, find out the scheduled dates of the sports physicals and their respective times. These dates are listed on the school’s website and discussed during team meetings. You will need to have a release form and health history form completed and signed by your parent’s/guardians before obtaining a PPE.
How long is my physical good for?
One calendar year. Once it expires you will be required to get a new physical to continue to participate (even if this occurs mid season).
I already played a sport in the fall, what forms do I need to go out for one in another season?
You need a release form and "UPDATE" form – short version of health history as long as your physical is up to date. This can be obtained during team meeting, on school’s website or nurse’s office.
I want to hand in my pre-participation physical in the summer, when and where do I drop it off?
Submit the completed forms directly to the school nurse during regular summer school hours. If summer school is over then wait until the August dates for school physicals are being held and bring during this time.
Why does the school physician need to review the PPE done by my family physician before I can participate?
This is required by the Department of Education. Make sure the physician completes all sections of the PPE or it will be returned to you. The school nurses are not permitted to complete parts of the exam omitted by your family physician.
What forms are necessary to hand in to try out for a sport and where can I get all the necessary forms?
You will need:
1. An athletic release form.
2. An Athletic Pre-Participation Physical Examination Form; Part A and B.
Team meetings are the best place to get all the forms and information. Forms can be also obtained at the nurse’s office or on the school’s website at Athletic Forms .
I signed up for one sport, but want to go out for a different one in the same season. What should I do?
Check with both coaches involved and make sure it’s possible. If possible, you need next to submit a new release form with parent signature indicating the new sport.
What sports are offered for males?
FALL – football, soccer, cross country, cheerleading
WINTER – basketball, wrestling, bowling, swimming, track, cheerleading
SPRING – baseball, tennis, volleyball, track, golf
What sports are offered for females?
FALL – cheerleading, tennis, cross country, field hockey, soccer, volleyball
WINTER – cheerleading, basketball, swimming, bowling, track
SPRING – softball, track, lacrosse, golf
When do sports seasons begin?
FALL – generally the third week of August.
WINTER – bowling / swimming start in mid November. All other winter sports start the day after Thanksgiving.
SPRING – the first Friday in March.
Note: During the summer, all programs can unofficially practice. Many of them do. Be sure to contact the appropriate coach to see what the summer schedule might require.
How do I know if I am academically elgible?
Check the Academic Eligibility page. See the Athletic Director if you have problems interpreting the policy.
I came in late to school the day of a game, can I still play in the game?
If you come in after 9:00AM, you must have a doctor’s note. Any questions, you may seek permission from the Athletic Director.
I have some demerits; can I still practice/play in my sport?
Yes, as long as you do not exceed 9 demerits. At 10, you are ineligible and must stay for central detention to eliminate one.
I went home sick from school, am I permitted to come back to participate in athletics?
No. Anyone sent home by the school nurse due to illness/injury may not return for practice or competition.
